excessive daytime sleepiness, cataplexy, sleep paralysis, and hypnagogic hallucinations (1). A nocturnal polysomnogram, followed by the multiple sleep latency test is suggested for the diagnosis. These tests, performed at a sleep disorders clinic, confirm the daytime sleepiness by showing a short sleep latency of usually less than 5 minutes, as well as an abnormally short latency prior to the first rapid eye movement (REM) period (2). HLA typing also helps the diagnosis. However, the tests often have false-negative and false-

Hypocretin (orexin) deficiency in narcolepsy and primary hypersomnia.
The discovery that hypocretins are involved in narcolepsy, a disorder associated with excessive daytime sleepiness, cataplexy, and unusually rapid transitions to rapid eye movement sleep, opens a new field of investigation in the area of disorders of sleep and activation. Hypocretin-1 (hcrt-1) and hypocretin-2 (hcrt-2) (also called orexin-A and orexin-B) are newly discovered neuropeptides proce...

STUDY OBJECTIVES Cerebrospinal fluid (CSF) hypocretin-1 levels and their relationship with the clinical characteristics of narcolepsy without cataplexy have not been well elucidated. Our aim was to examine whether clinical characteristics vary with CSF hypocretin-1 levels among narcoleptic patients without cataplexy. DESIGN Clinical features, variables on the multiple sleep latency test, and ...
متن کاملThe role of cerebrospinal fluid hypocretin measurement in the diagnosis of narcolepsy and other hypersomnias.
CONTEXT Narcolepsy, a neurological disorder affecting 1 in 2000 individuals, is associated with HLA-DQB1*0602 and low cerebrospinal fluid (CSF) hypocretin (orexin) levels. OBJECTIVES To delineate the spectrum of the hypocretin deficiency syndrome and to establish CSF hypocretin-1 measurements as a diagnostic tool for narcolepsy. DESIGN Diagnosis, HLA-DQ, clinical data, the multiple sleep la...
متن کاملDiurnal variation of cerebrospinal fluid hypocretin-1 (Orexin-A) levels in control and depressed subjects.
BACKGROUND Hypocretins, excitatory neuropeptides at monoaminergic synapses, appear to regulate human sleep-wake cycles. Undetectable cerebrospinal fluid hypocretin-1 levels are seen in narcolepsy, which is frequently associated with secondary depression. Shortened rapid eye movement latency is observed in both narcolepsy and depression. Cerebrospinal fluid hypocretin-1 levels have not been repo...
متن کاملNocturnal rapid eye movement sleep latency for identifying patients with narcolepsy/hypocretin deficiency.
IMPORTANCE Narcolepsy, a disorder associated with HLA-DQB1*06:02 and caused by hypocretin (orexin) deficiency, is diagnosed using the Multiple Sleep Latency Test (MSLT) following nocturnal polysomnography (NPSG). In many patients, a short rapid eye movement sleep latency (REML) during the NPSG is also observed but not used diagnostically. OBJECTIVE To determine diagnostic accuracy and clinica...
